If you are visiting us for the first time we will need the following from you prior to one on one consultation:
1. Updated health questionnaire (recent surgeries, therapies, health conditions, etc.)
2. Updated medication history (please bring the list and dosage of current medications taken)
3. Names and contact number of current medical and dental providers.
4. Updated dental history, dates of recent dental extractions, restorations, examinations.
5. All dental insurance information in case we need to submit a pre-authorization on your behalf.
